NEW YORK—A woman died on Wednesday, Dec. 14, when an elevator malfunctioned and rose when she was stepping in, crushing her between the elevator and wall, according to ABC News.

Manhattan Borough President Scott Stringer issued a statement calling on the Department of Buildings to “mount a full investigation, so this kind of tragedy will never happen again.”

The accident happened at 285 Madison Ave.

Stringer referred to a report his office released in 2008 called “Dangerous Neglect.” It found that three-quarters of New York City Housing Agency elevators were failing routine inspections between 2003 and 2008.

It included a recommendation that information about elevators should be publicly accessible so people know which ones are safe.

“The city must put in place every mitigation possible,” Stringer concluded, “to ensure that a tragedy like this is prevented in the future.”
