Tragedy struck on the set of the indie film “Rust” when Alec Baldwin was handed what he thought was a “cold” prop gun, shooting and killing his cinematographer Haylyna Hutchins and wounding his director, Joel Souza. But behind the accident are a number of questions regarding the overall safety of this set and what could have been a largely avoidable incident.
